Important Information

• Reporting of Work-Related Injuries & Illness

  In order to ensure that you receive all of the benefits that you are entitled to, you must make sure to report all work-related injuries or illnesses immediately to your supervisor or other designated official in your department. If you do not know who this individual is please contact your supervisor to identify the appropriate individual for reporting injuries and provide you with a point of contact for issues through your injury or illness.

  You are also encouraged to use the Accident Reporting System (ARS). Calling ARS assists in ensuring that your benefits are available to you as soon as possible and helps with tracking injuries and illnesses.

• Maintaining Communication During Any Workers’ Compensation Absence

  During any work-related absence, it is important that you follow campus’ rules for maintaining contact and submitting any needed medical information. Such rules and submissions may be in addition to any other requirements to submit medical information. Maintaining contact and communication will help ensure that you receive timely information about your benefits and any changes thereto, such as any requirement that you return to work on light duty.

• Reporting Fraud & Abuse

  Fraud and abuse of Workers’ Compensation is not only illegal but expensive. Moreover, it greatly impacts the employees who remain at work, because they can find themselves subject to longer work hours, reduced time off and additional work assignments in order to cover for individuals who are improperly absent from work. If you suspect fraud or abuse, please contact the New York State Workers’ Compensation Fraud Inspector General at 800-367-4448.

• Near-Miss Incidents & Reporting

  Definition:  a “near miss” is an incident in which no property was damaged and no personal injury was sustained, but where, given a slight shift in time or position, damage or injury easily could have occurred.

  The same things that cause near misses cause incidents. By monitoring near misses, you’re also fulfilling your goal to reduce workplace accidents.

  Near Miss Examples:

  • A worker slips on condensation that had dripped onto the floor but doesn’t fall (this time).
  • A worker trips over an unmarked step and falls without injury.
  • Your leg was momentarily caught between a pallet of materials and a support structure (no injury).

• Accident Reporting
  • All supervisors should take Accident Investigation Training
  • Brochures are now available to provide guidance on the reporting of accidents.
  • If an accident occurs, report it immediately to your supervisor, regardless of how big or small it may be
  • Supervisors must complete a Supervisors Accident Investigation report and return it to EH&S
  • Complete either the “Employee Accident Report” or the “Student and Visitor Accident Report” (CS-13) and return it to Human Resources.
  • Seek medical attention if needed
  • All incidents regarding state-owned vehicles must be reported by notifying University Police 518-564-2022.

  Safety Concerns may be reported anonymously by completing a form and returning it to Environmental Health and Safety or fax 518-564-5082. All reports will be investigated.
