History of the Eastern Adirondack Health Care Network
The Eastern Adirondack Health Care Network originally began in 1993 as an informal Primary Care Coalition in Essex County consisting of three members. In 1994, Champlain Valley Physicians Hospital received a Rural Health Network Demonstration grant from New York State Office of Rural Health to establish the Eastern Adirondack Health Care Network in Clinton and northeast Essex Counties.
Development of a representative membership within the service delivery area began and the Network grew to seventeen members. In 1995, SUNY Sponsored Research and the Northern New York Alzheimer's Disease Assistance Center became the contract agency for the grant. It was at this time that the Network began to establish non-profit status and articles of incorporation and by 1998, the Network had evolved into a formal operating network with established bylaws and committee structures.
