Incident Report
for Student Clubs/Organizations, Fraternities/Sororities and Athletic Teams

Incident reports will be submitted to The Dean of Students office. Please note that any contact information you provide will be kept CONFIDENTIAL. Providing contact information that can be used for follow up greatly increases the probability that we will be able to accurately address the issues listed in the report. Please be as specific as possible when describing the incident. Vague responses such as (an organization was hazing and it needs to be addressed) will not be helpful in helping us address the issue.

Who was involved: Please list specific names, parties, organizations, what they looked like, what they were wearing, the colors of their letters, etc. that can help us identify who was involved.

What happened: Please be sure to be very specific. Please provide details and as much information as possible on what happened. For example: “They were hazing,” or “They were fighting,” or “They had an open party” is not enough information for us to follow through on the incident report.

When: When did the incident occur? Please be specific such as Thursday September 30, 2010 at 2:00AM. “Last” week or “a couple of days ago” is not specific enough.

Where: Be specific: On the side porch of 999 Brinkerhoff St, a white house with blue shutters and an American flag on the left pillar.

Other: Please provide any additional information that could be helpful such as who else may have information etc.
