Instructions for Completing ATR Form

Authorization to Recruit

  1. Search Number: This is the CPJ # or PJ # that is assigned by Human Resources. This number is not assigned until the Human Resource Associate has reviewed all criteria and attachments.

Background Information

  1. Department Name: Please indicate hiring department here.
  2. Account Number: Plattsburgh State account number(s) where position is to be charged.
  3. Account Title: Please indicate title that corresponds with account number(s) here.
  4. Source of Funds: Description of funding source. If unknown, contact appropriate administrator or the Budget office.
  5. Reason for Vacancy: Identify purpose for vacancy (e.g. resignation, retirement, etc.).
  6. Previous Line Number: Please indicate here, if new position leave blank.
  7. Previous Incumbent: Please indicate name of previous incumbent here, if new position leave blank.
  8. New Line Number: Leave blank, this will be assigned by Budget office.
  9. Previous Incumbent’s Budget Title: Approved SUNY budget title, not descriptive campus title.
  10. Date of Vacancy: First day position is vacant.
  11. Remarks: Explanation or additional information that may be helpful in the recruitment process.

Recruitment Request

  1. Type of Search: Please select one appropriate box, reference Search Process Manual for explanations.
  2. Attachments:
  • Unclassified – verify that all of the appropriate items are included with Authorization to Recruit.
  • Classified – list of job duties and job description are the only required attachments.
  1. Type of Position: Please check one (call HR if clarification required). When work obligation & appointment periods are not the same, both must be specified.
  2. Budget Title: Must correlate with approved SUNY System Administration titles. (Reference the Job Specification Reference Guides or contact Human Resources)
  3. Campus Title: Descriptive title applicable to Plattsburgh State University of New York.
  4. Salary Range: Unclassified only – Use annualized full-time equivalent (FTE) salary that falls within contractual guidelines. Note: If range is not indicated, the Administrator will be limited to the salary stated.
  5. Salary Grade/Level: Reference Job Specification Reference Guides (contact HR if clarification required).
  6. Appointment Attribute: Please indicate full-time, part-time or hourly here. If part-time or hourly, state number of hours per week.
  7. FTE: If full time use 1.0. If part-time academic, please refer to chart below, otherwise will be determined by Budget office based on work obligation.

  1. Work Schedule: For non-academic positions only.
  2. Shift: Classified only – Please check box if non-standard work day.
  3. Type of Appointment: For unclassified positions, reference Board of Trustees policies for definitions, and indicate duration of appointment. Contact Human Resources for Civil Service positions.
  4. Approvals/Signature Route: Route in order indicated. Upon completion of signature approval section, HR will distribute copies to offices indicated on distribution list found at the bottom of the ATR.
