Freshmen

SUNY Supplemental Application for Freshmen - If you have applied through the SUNY Application, you are required to submit the SUNY Supplemental Application for Freshmen. The supplemental can be completed by logging into your SUNY account.
Within the SUNY Supplemental, you will complete the essay required for admission. While not required, we strongly encourage you to submit the name of your counselor. If he or she is planning to write a letter of recommendation on your behalf, our office will send an e-mail requesting it automatically.
Please note: If you have applied using the Common Application, you do not need to submit the SUNY Supplemental Application for Freshmen.
Transfers
- Transfer Supplemental Application Form - If you have applied through the SUNY Application, you are required to submit the Transfer Supplemental online in your MyPlattsburgh account. You will be sent an e-mail with a link to the online Transfer Supplemental once you have applied and your application has been processed.
Within the Transfer Supplemental, you will complete an essay and identify the professor, academic advisor, or teacher who will be writing a letter of recommendation for you. Our office will then send an e-mail to the designated person requesting that he or she submit a letter of recommendation on your behalf.
Please note: If you have applied using the Common Application, you do not need to submit the Transfer Supplemental Application Form. - Request for Mid-Term Grades Form - In some cases, the Admissions Office may request your mid-term grades before reviewing your application for admission. If your college produces mid-term grades, simply print them out and staple them to this form. If not, please have your professors list your grades and sign their names on the form. - SUNY Student Transfer Appeal Process Form - The SUNY Student Transfer Appeal process is designed for any student in a SUNY associate degree program who does not agree with the decision of the college regarding acceptance and/or placement of credit earned elsewhere within SUNY. - EOP Transfer Checklist - Transfer students who are interested in participating in the Educational Opportunity Program (EOP) should review the EOP Transfer Checklist carefully as it details the documents needed to gain admission as an EOP student. EOP provides additional support for those students who are financially and academically qualified and is usually reserved for those students who were previously enrolled in an EOP-type program (such as EOP, HEOP, SEEK, or College Discovery). - EOP Transfer Verification Form - Transfer students who are interested in participating in the Educational Opportunity Program (EOP) must complete the transfer verification form. Readmission
- Application for Readmission - This form must be completed by students seeking readmission to SUNY Plattsburgh. For more details about applying for readmission, please visit the Readmission page. Graduate
- Application for Admission for Graduate Study - Applications to graduate programs at Plattsburgh are processed by the Graduate Admissions Office, located on the first floor of the Kehoe administration building. Download Application for Admission to Graduate Study (PDF file size 684Kb)
